Governing Body Membership and Register of Interests
The CCG keeps a register of interests for the members of the Governing Body, in order to identify and manage any conflicts of interest that may arise in the course of decision making when undertaking CCG business. These registers are reviewed regularly and updated as necessary.

Practice Staff Register of Interests
The CCG keeps a register of interests for all GP Partners and Practice Staff involved with the business or decision-making of the CCG (as defined by the CCG policy) in order to identify and manage any conflicts of interest that may arise in the course of decision making when undertaking CCG business. These registers are reviewed periodically and updated every six months on the website.
Staff have given their consent to publish this information. Where consent to publish has not been given, the Conflict of Interest Guardian reviews and considers whether the rationale for this is acceptable.

CCG Staff Register of Interests (Band 8a and above)
The CCG keeps a register of interests for all staff in order to identify and manage any conflicts of interest that may arise in the course of decision making when undertaking CCG business. These registers are reviewed periodically and updated every six months on the website.
The CCG publishes declarations of senior staff who are deemed to have a “decision making” role. We have used Agenda for Change Band 8a and above as the threshold for publication.
Staff have given their consent to publish this information. Where consent to publish has not been given, the Conflict of Interest Guardian reviews and considers whether the rationale for this is acceptable.

The CCG keeps a register of gifts and hospitality offers made to staff and Governing Body members, which is reviewed and updated regularly.

Procurement Decisions and Contracts Awarded Register
The CCG keeps a register of procurement activity undertaken containing the outcomes and values of the procurements and whether any conflicts of interests were noted. This is reviewed and updated regularly.

Breaches of the Standards of Business Conduct Policy
In line with CCG Policy and NHS England Guidance, the CCG is required to report and publish anonymised details of all breaches of the policy following investigation.
